A series of coloured enamelled brooches were worn by the nursing sisters to indicate various grades of nursing qualification.

Maybee he was friends with a German nurse

Silver txt, gold txt.... not sure of the rank system... but I'm sure someone is

F&R Will be the maker Frank & Reif of Stuttgart and the Ges. AT is a shortened phrase to indicate it is a patented design.
The awards have different nursing grades written on them As well at Helferin there is Schwesternhelferin, Samariterin and Schwesternschaft [though this one may be weimar period] They are also found without the Swaz. for pre nazi era as well. The schwestern grades have an additional wide silver border on the pin.
As to how he got it, who knows! ;)

Two pictures with the brooch for DRK nurses .... mine is in the lower picture and is an original too.
The smaller brooches with Helferin/Samariterin/Schwesternhelferin are only used for 'ranks' below the nurse.
